If you’d prefer to fill out the School of Spiritual Direction application offline, please follow these instructions:
- Download and complete the Application
- Have recommenders complete the recommendation forms in either of the following methods:
- Recommendation forms can be completed online by having each of your three recommenders (Academic, Ministerial, Personal) visit this web address: https://CQCenterQuest.org/recommendation
- Recommendation forms can be completed offline by downloading the recommendation forms (Academic, Ministerial, and Personal) and having your recommenders send the completed forms to the address shown below.
- Please note that CenterQuest has to receive all three recommendation forms no later than two weeks from the date of application submission.
- Send the completed application back to us, along with your non-refundable, non-transferable application fee of $ 150 to:
